There are three options to obtain a printed SCEC order form and catalog:

• Download the form (PDF, 92 K).
  Download the form and catalog (PDF, 940 K).
• View an image of the form and print it from your browser.
• Call 213-740-5843, fax 213-740-0011, OR e-mail scec@usc.edu and request a form be mailed or faxed.

When you have a printed form, enter the necessary information and then fax or mail with your payment.

SCEC Accepts Visa, Mastercard, or Discover, as well as checks or money orders.
